Application forms for the reconstruction of earthquake-damaged buildings of Sisak-Moslavina County, Karlovac County and Zagreb County area

• Form 1 - Request for repair of building structure, i.e. complete renovation of individually protected cultural property (organized renovation)
• Form 2 - Request for removal of the building and construction and equipping of the replacement family house
• Form 3 - Request for financial assistance
• Form 4 - Application for the construction of a multi-apartment or residential-office building in the area where the disaster was declared
• Form S - Application for housing care

• to the Ministry of Physical Planning, Construction and State Assets in Zagreb, Ulica Republike Austrije 14, personally at the clerk's office or recommended by post with the indication: “Request - reconstruction after earthquake”;
• at the Reconstruction information Centre, Ulica kneza Mislava 2 in Zagreb;
• in the displaced offices of the Ministry:
